Foster Parent Waiver and Release

Please read the following statements; they constitute the conditions under which you would be volunteering at the League should you be accepted as a foster parent. Please note that this is an application and is not a guarantee of placement.

I certify that all information that I have provided on this application is true and complete to the best of my knowledge. Any misrepresentation, or omission of facts called for in this application may result in rejection of my application as a foster parent or dismissal from the foster care program.

I understand that if I am selected as a foster parent, I will be devoting my time and services to the League on a purely voluntary basis and primarily for my own benefit. I will serve with no expectation of compensation for my services or reimbursement for any expenditure which I incur for the care and treatment of my foster animal(s). I understand that my foster care assignment may end at any time without notice or cause and that the disposition of any animal(s) is at the sole discretion of the Animal Rescue League of Boston’s management.

If, selected as a foster parent, I agree to abide by all of the League’s rules and regulations. This includes, but is not limited to:

Providing adequate food, water, shelter and kind treatment for the animal(s) at all times.

Isolating the animal(s) upon receiving them into my care to minimize the possibility of my own animals becoming ill.

A reasonable, continuing effort to monitor the animal(s) to assure proper health so that their possibility for adoption once returned to the Animal Rescue League of Boston is maximized. If the foster animal becomes ill or is injured, agreeing to bring it to the Animal Rescue League of Boston for treatment or another designated veterinarian.

Keeping foster animal(s) under control, and not violating any local leash laws or proper scooping regulations. Any fines assessed by municipal authorities will be my responsibility.

I give the League permission to investigate all pertinent information and references concerning my foster parent application. I hereby release said employers, companies, schools, or persons from all liability for any damage, both legal and otherwise for issuing information.

If selected as a volunteer, I agree to sign a release and “hold harmless” agreement arising from or related to my foster care activities.

If selected as a foster parent, I agree to maintain the confidentiality of all proprietary or privileged information of the League to which exposed while serving as a foster parent, whether this information involves a single staff, volunteer, client, animal, or other person or involves overall League business. I also acknowledge that failure to maintain confidentiality would be cause for dismissal from the foster care program.

In consideration of my acceptance as an Animal Rescue League of Boston Foster Parent, my signature below indicates that I have read, understand, and agree to the following:

I acknowledge and have been advised of certain communicable diseases that can be contracted from interacting with animals, and safety precautions to prevent contracting such communicable diseases. Such diseases principally include, but are not limited to, Rabies, Cat Fever, Hepatitis, Hookworm, Ringworm, Intestinal Worms, Mites, and Mange.

I acknowledge and have been advised of the risk of injury from interacting with animals, including bites, and scratches, or other injuries from animals, and agree to report any such injury immediately to my supervisor.

I authorize the Animal Rescue League of Boston to seek emergency medical treatment for me in case of an accident, injury, or illness.

I agree to abide by all policies and procedures of the Animal Rescue League of Boston.

I hereby agree to release, indemnify and hold harmless the Animal Rescue League of Boston, its directors, officers, employees, and agents from any and all claims, damages, and liability, including, but not limited to, any loss or injury to me and/or my personal property arising from or related to my activities as an Animal Rescue League of Boston Volunteer under this agreement or for any negligent act or omission by the Animal Rescue League of Boston, its directors, officers, employees, and agents. In connection with my activities as an Animal Rescue League of Boston Volunteer, I further agree to release, hold harmless and indemnify the Animal Rescue League of Boston, its directors, officers, employees and agents, from any and all claims, damages and liability, including, but not limited to, any loss or injury to me and/or my personal property arising from my riding as a passenger in any motor vehicle owned or leased by the Animal Rescue League of Boston and operated by an employee or authorized agent of the Animal Rescue League of Boston.
